Below are forms that can be printed and set to the Council to report change of circumsnaces and other changes:

 

 Authority to Discuss with a Third Party

    (Used to enable someone else to speak to the Council on your behalf.)

 

 Authority to Disclose financial information

    (Used to enable the Council to speak to a company regarding your financial affairs.)

 

 Change of Address Form (Inc. Income Changes)

    (Used to report a change of address as well as a change in income to the Council.)

 

 Change of Address Form

    (Used to report a change of address to the Council.)

 

 Change of Circumstances Form

    (Used to report a change of circumstances to the Council.)

 

 Certificate of Earned Income

    (Used to give to your employer for completion of proof of earnings, when payslips are not available)

 

 Review Form
